Even though DPS places a heavy focus on safety, accidents may occur. What matters is how we respond. DPS pledges to respond in a way that ensures a positive outcome for all.

New Employees

We are excited to have you on our team! Your DPS Sourcing Specialist will give you an access code to complete your training and safety videos.

Supervisors/Managers

Should you experience a workplace accident, be sure to fill out the Supervisor Incident Report form and distribute the Employee Incident Report to the injured employee.

These forms should be scanned and returned to IncidentReport@oppinc.com

Supervisor Incident Report

It is imperative that ANY incident or accident be reported immediately. If there are any witnesses to the incident/accident, please have them complete a statement – who, what, when, where, why. Please take photos of the area the incident/accident occurred, if possible, and send to IncidentReport@oppinc.com
